Connected Wellness

NexJ Connected Wellness provides a set of cloud-based, patient-centered applications that enable patients and providers to work together across the continuum of care.

Accessible through the web or on a mobile device, the connected applications of NexJ Connected Wellness include:

  • Health Coach: Enables personalized wellness and supports positive behavior change between an individual and a health coach through a mobile device.
  • Chronic Disease Management: Enables chronic disease management through a provider and patient portal and mobile devices.
  • Disease Screening: Enables targeted screening for diseases across a population.

In addition, NexJ has just entered into an agreement with the Beth Israel Deaconess Medical Center, a Harvard Medical School affiliate, to digitize the Passport to TRUST program, which structures the doctor-patient interactions. Through NexJ Connected Wellness, patients will be able to access a “Trust Passport”, which provides a blueprint for comprehensive care that is unique to each patient. The Trust Passport helps facilitate two-way communications between patients and physicians, so that providers and patients can communicate across disparate medical records, deliver care instructions beyond the office visit, and support patients in their treatment, ultimately reducing readmission rates and improving overall health care outcomes.

The applications of NexJ Connected Wellness effectively motivate patients to manage issues related to lifestyle, mood, diet, exercise, conditions, and chronic disease, so they can improve outcomes and live happier, healthier lives.
